Sixty years of safety data, pennies a day, and the single most-studied metabolic drug in existence. Metformin improves insulin sensitivity and blunts the glucose spikes that drive afternoon crashes and slow fat regain.

It is not a dramatic weight-loss drug and we will not sell it as one — typical trial effects are a few kilograms, not twenty. It is a metabolic workhorse: cheap, boring, and durable. Sometimes boring is the prescription.

“Appetite was never a character flaw. It is a signal — and there is medicine now that turns the volume down. My job is making sure the muscle survives the cut.”

The evidence

Why Metformin.

Six decades of evidence

Approved since the 1950s in Europe and 1995 in the US, metformin is first-line therapy for type 2 diabetes worldwide, with the Diabetes Prevention Program showing a 31% reduction in progression to diabetes in high-risk adults.

The longevity conversation

Observational data and the planned TAME trial have made metformin the most-discussed longevity candidate in medicine. The human anti-aging evidence is not settled — we tell you that plainly, because the honest version is still a good drug.

Plays well with the rest of the plan

For files coming off a GLP-1 or holding maintenance, metformin is a common, low-cost metabolic floor your provider may recommend.

Q & A

Common questions.

What is metformin? +
A generic biguanide that lowers hepatic glucose output and improves insulin sensitivity.
How is it taken? +
A daily tablet, usually started low and titrated to minimize stomach upset. Extended-release versions are gentler.
What are the side effects? +
GI upset early on for some people, and rare B12 depletion with long-term use — worth a periodic lab. Serious risks are rare and screened for.
Who shouldn’t take it? +
Significant kidney impairment is the main exclusion. Your provider checks kidney function before and during treatment.
